The full plan: an ordered list of steps plus any refusals.

steps is only safe to execute when refusals is empty; a caller that finds refusals should surface them loudly (the sink fails the batch, naming the column) rather than applying a partial plan.

The actions to apply, in order.

§refusals: Vec<Refusal>

Changes the planner refused; non-empty ⇒ do not execute.
